This community service program aims to improve family members' knowledge of providing support and care for patients with chronic diseases. The program employed an educational and mentoring approach involving 15 participants in Pamboborang Village, Majene Regency, Indonesia. Data were collected using pre-test and post-test assessments. The results indicate an increase in the proportion of participants with a good level of knowledge, from 66.7% to 80%, while the proportion with a poor level of knowledge decreased from 26.7% to 0%. The Wilcoxon signed-rank test yielded a p-value of 0.034, indicating a statistically significant change in participants' knowledge levels following the educational intervention. These findings suggest that family-centered education has the potential to improve family members' knowledge and strengthen their capacity to support the home-based care of patients with chronic diseases.
